Our Living Well Service reaches into many of our hospice services, helping patients to achieve personal goals after they’ve been diagnosed with a life-limiting illness.

For some, Living Well will be the front door to Trinity as we support them through tailored clinics and group sessions to live as well as possible and achieve ambitions after their diagnosis. Whether you’d like to get back to work following treatment, improve your fitness or cognitive abilities or just need some wellbeing advice – we’re here to support you to live your best quality of life through rehabilitation and self-management.

Living Well brings together many of Trinity’s services and specialist clinicians in palliative care, counselling, physiotherapy, complementary therapy, occupational therapy and clinical psychology to support people from the point of diagnosis to get them physically and mentally to a place they want to be.

If you choose to attend, based on what is important and meaningful to you, you will be offered the opportunity to engage with the programme to develop a personalised treatment plan.

Ask your consultant, GP or healthcare professional about a referral to our Living Well Service.

I found the sessions very beneficial, learning new ways of coping. I enjoyed trying out new ideas and treatments with staff who were very understanding and made me feel comfortable in expressing my feelings.


We also hold support groups for carers, helping them to gain confidence and build networks during what can be a difficult time. We know that relatives of someone who has been diagnosed with a palliative condition may experience their own anxieties, pressures and loneliness.

Our Wellbeing groups offer a safe, welcoming space to support people as they care for the ones they love, providing practical, interactive and informative support.


The sessions have really helped me through a difficult time and gave me something soothing and enjoyable to look forward to.


Who can access the Living Well Service?

Any adult with a life-limiting illness can be referred to the Living Well Service at Trinity. Patients must be referred by a health care professional; this could be their Consultant, GP, District Nurse or social worker.

Once we have received a referral, someone from the Living Well team will be in touch to invite you to one of our Introductory Days. These are a great way to meet the team and learn about the many different sessions and support available. It's also a great chance to look around and ask any questions.

Is it for all ages?

All ages, all interests, all backgrounds. Patients can range from their 20s to their 90s and live anywhere on the Fylde coast. Our staff and volunteers promise everyone a very warm welcome.
