Community team

Our Community Palliative Care nurses provide an additional level of specialist palliative care in patients’ own homes. As referrals are made by GPs and the hospital care team, ask your GP if you think you or a loved one may benefit from this service.

Hospital team

If you or a loved one is a patient at the hospital you may be cared for by our Palliative Care team, who work in partnership with nursing and medical staff to improve quality of life for those with terminal illness.

Linden Centre

The Linden Centre provides counselling and support to adults, children and young people living in Blackpool, Fylde and Wyre.

Hospice at Home

A service for those in the final months of life, referrals are made by health professionals involved in a patient’s care. Ask your GP or consultant about being referred to the team for this specialist overnight support.

In-Patient Unit

Admissions are made following referrals from the hospital, from GPs or other health professionals. Some patients come to us for a review of pain medication or to ease symptoms such as nausea, and are then discharged home.

Lymphoedema

Our clinic is run as an out-patient service Monday to Friday and referrals come from GPs and the hospital.

Complementary therapy

You must be a current Trinity Hospice patient or their carer to use our complementary therapy services. Ask our staff for more information about referrals.
