Trinity is committed to providing accessible, high quality Specialist Palliative Care for everyone affected by an advanced, progressive life limiting illness. Once a patient is referred to Trinity they are able to access the Community Team, In-patient Unit, Living Well Service, Linden Centre, Lymphoedema Service and Complementary Therapy.
As referrers, you can then ensure that your patient can access all or some of the services that they are entitled to.
Referral Criteria
Patient has a progressive, life limiting diagnosis
&
Has complex problems that cannot be adequately addressed by current healthcare team
&
Has a reasonable understanding of their illness, where has capacity
&
Accepts referral to specialist palliative care services
All patients referred to Trinity must live on the Fylde coast.

Attach any relevant documentation with the referral i.e. list of current medication, relevant out-patient clinic or hospital discharge letters, oncology annotations, etc.
