How can the specialist palliative care team in the hospital help?

The specialist hospital Palliative Care team is dedicated to supporting patients with life-limiting illnesses across all areas of the hospital. We collaborate closely with other specialties to enhance the care provided to patients and those important to them, ensuring a holistic approach to care and treatment.

Our Services Include:

  • Expert Symptom Management: Our specialist nurses are highly experienced in palliative care, providing support and advice on complex symptom management, spiritual care, and psychological support.
  • Comprehensive Family Support: We offer guidance and assistance to families during challenging times, ensuring they receive the necessary support.
  • Interdisciplinary Collaboration: We work alongside other specialties to provide integrated and comprehensive care.
  • Continuity of Care: Our support doesn't end when a patient is discharged. We can access a wide range of other specialist services based on a patient's wishes and needs, ensuring they have everything required to live well with their illness.

Who Are We?

Our hospital's Palliative Care team is composed of a group of dedicated professionals, including:

  • Palliative Medicine Consultants
  • Specialty Doctors
  • Trainee Advanced Practitioners
  • Clinical Nurse Specialists
  • Associate Clinical Nurse Specialists
  • NVQ3 Nurses

These team members collaborate closely with other departments within the hospital to enhance the care provided to patients and their families or carers.

Our Availability:

  • Operating Hours: 7:30 AM to 5:30 PM, seven days a week (Monday to Sunday), including all Bank Holidays.

What Does Trinity's Specialist Palliative Care Team Do?

Our team at the hospital works in partnership with other hospital teams to provide comprehensive support and care, including:

  • Expert symptom Guidance: Offering advice on medications that may be more effective in alleviating symptoms such as pain, nausea, or breathlessness.
  • Psychological and Emotional Support: Providing complex psychological and emotional support for patients and their families.
  • Future Planning Advice: Helping patients, families, and carers make informed decisions about the future.
  • Integrated Care Approach: Collaborating with a patient's wider care team, including GPs, District Nurses, and Consultants, to ensure a cohesive approach to care.
  • Service Assessments: Evaluating whether patients and relatives would benefit from Trinity's other services, such as the In-patient Unit, Complementary Therapy, and Counselling at the Linden Centre.
  • Referrals to Other Providers: Facilitating referrals to other providers if needed, such as social services, occupational therapy, physiotherapy, or benefits advice.
  • Education for Healthcare Professionals: Educating other local healthcare professionals to help them better support the patients they are caring for.
