    The information you give on this application form will allow us to assess your suitability for voluntary work at Trinity Hospice.
  • Note: If you are enquiring about a placement as part of The Duke of Edinburgh's Award (DofE) for someone over the age of 14, please contact your preferred shop to determine current vacancies.

    We’d love to keep you updated on how your support is making a real difference to our patients and we may contact you by post or telephone to let you know about our services and other ways you can get involved. Please let us know if you are happy to hear from us by email and text.

  • Referees

     
    Two references are required to support all volunteer applications. 

    Please provide the names and addresses of two referees who have known you for at least 12 months, and tell us how they know you. Referees can be a previous/recent employer, course lecturer, someone of standing in the community or a long-standing friend, but they must not be related to you.
  • Criminal Offences

    Applicants must not withhold information of convictions which, for other purposes, are spent. Previous convictions will not necessarily prevent you from becoming a volunteer. Please answer the questions below. All information provided to us will remain strictly confidential.

  • Data Protection Notification

    The information you have provided in completing this form will be used to process your application for volunteering with us. If you do not commence volunteering at Trinity your personal information will be confidentially destroyed within one month. If you become a volunteer at Trinity, your personal information will be held for up to 6 years. We will keep the information you have supplied to us confidential and will not divulge it to third parties without your permission, except where required by law. Access to your personal information is restricted to only those who need to view it in the course of their work.
