Referrals Information
Trinity is committed to providing accessible, high quality Specialist Palliative Care for everyone affected by an advanced, progressive life limiting illness.
Today, Trinity’s family of services has grown. Once a patient is referred to Trinity they are able to access the Clinical Nurse Specialist Team, In-patient Unit, Day Therapy Unit, Linden Centre, Lymphoedema Service and Complementary Therapy.
As referrers, you can then ensure that your patient can access all or some of the services that they are entitled to.
Referral Criteria
Patient has a progressive, life limiting diagnosis
&
Has complex problems that cannot be adequately addressed by current healthcare team
&
Has a reasonable understanding of their illness, where has capacity
&
Accepts referral to specialist palliative care services

Please download and print the form. Please feel free to photocopy as many as you need.
Completed referrals should faxed to 01253 595654 or posted to Trinity Hospice, Low Moor Road, Bispham, Blackpool, FY2 0BG.
Please attach any relevant documentation with the referral
i.e. list of current medication, relevant out-patient clinic or hospital discharge letters, oncology annotations, etc.
